When Does New Employee Coverage Begin?

One of the most common questions from new employees is: "When does my health insurance start?" The answer depends on your plan's waiting period — the period between an employee's hire date and their eligibility for coverage.

Under the Affordable Care Act, employer-sponsored plans cannot impose a waiting period longer than 90 days. In practice, most Massachusetts employers use one of these common waiting periods:

  • First of the month following hire: Coverage begins on the 1st of the month after the employee's start date
  • First of the month following 30 days: A one-month waiting period before coverage begins
  • 60-day waiting period: Coverage begins after 60 days of employment
  • Immediate coverage: Some employers offer coverage starting on the first day of employment

The Enrollment Window

New employees typically have 30 days from their eligibility date to enroll in coverage. Some plans allow up to 60 days. If an employee misses this window, they must wait until the next open enrollment period — unless they experience a qualifying life event.

It is critical that employers communicate the enrollment deadline clearly during onboarding. A missed deadline can leave a new employee without coverage for months, which can create significant hardship and HR complications.

Required Notices for New Employees

Federal and Massachusetts law require employers to provide several notices to new employees regarding their health benefits:

  • Summary of Benefits and Coverage (SBC): A standardized document summarizing each plan option, including costs and coverage details
  • CHIP Notice: Information about the Children's Health Insurance Program and premium assistance programs
  • COBRA Notice: Information about continuation coverage rights (required within 90 days of coverage beginning)
  • Exchange Notice: Information about the Health Insurance Marketplace and eligibility for subsidies

Dependent Coverage

New employees can typically enroll their dependents — spouse, domestic partner, and children up to age 26 — during their initial enrollment window. The same 30-day deadline applies to dependent enrollment. Employees who miss the window for dependents must wait until open enrollment or a qualifying life event.

Best Practices for New Hire Enrollment

  • Include benefits enrollment materials in your new hire packet on day one
  • Set a clear deadline and send reminders at 15 days and 5 days before the deadline
  • Offer a brief benefits orientation session (in-person or virtual) to walk employees through their options
  • Make a benefits advisor available to answer individual questions
  • Use a checklist to ensure all required notices are provided and documented
